IRA Qualified Charitable Distribution Confirmation

Thank you for your form submission. Please use the following information to complete your IRA Qualified Charitable Distribution gift to Baltimore School for the Arts.

1. Contact your IRA custodian to begin the process of making a gift from your IRA

2. Complete any instructions and follow procedures from your IRA custodian to request a “Qualified Charitable Distribution.”

• Please include the donor’s basic information (name, address, email, etc.) along with the check, so that BSA can thank the donor and send an acknowledgement for the generous contribution.

3. Provide your IRA custodian with the following information to complete the gift:

• BSA’s Tax ID#: 52-1174284
• Checks should be made payable to: The Baltimore School for the Arts Foundation
